Job Details

- Certifications & Education: BLS certification required.
- Experience: Minimum of 2 years of social work experience, including acute care or medical surgical environments.
- Schedule: 8-hour shifts, Monday through Friday, with a minimum of 40 hours per week. The start date is 11/16/2026, ending on 02/15/2027.
- Additional Responsibilities: Facilitate discharge and contingency planning, participate in care conference activities, document care delivery, educate patients and caregivers, review health history, and refer to other disciplines or resources as needed.
- Qualifications: Must have experience with discharge planning, documentation, and care coordination skills, along with familiarity with adult protective services, medication assistance, and community resources.
- Preferred Qualifications: Experience with electronic charting systems such as Epic, and familiarity with Washington state regulations and resources.
- Float: Float rotation may be required.
- Submission Requirements: Candidates must respond to previous employment at Providence facilities, supply two references covering at least one year within the last three years, and confirm minimum experience.
