Job Details
- Schedule: 8:00 AM to 4:30 PM, Monday through Friday, with a minimum of 40 hours per week.
- Required Qualifications: Graduate of an accredited school of social work, sociology, or related field; Licensed Clinical Social Worker (LCSW) in Montana; at least 1 year of clinical experience.
- Preferred Qualifications: Two years of hospital-based case management experience; optional ACM or CCM certification.
- Responsibilities include assessing patient needs, developing individualized care plans, facilitating discharge planning, coordinating resources, and advocating for patient rights.
- The role involves participation in multidisciplinary team rounds and case conferences, as well as promoting the organization’s mission, vision, and values.
