Job Summary:
Beverly & Addison Gilbert Hospital is seeking a Social Worker for a full-time position in their healthcare organization. This role involves working as part of the care coordination team to perform psychosocial assessments, provide supportive counseling, and coordinate discharge planning and services. The position requires collaboration with nursing, clinical teams, and external agencies to support patient needs across adult, geriatric, pediatric, and medical-surgical populations.
Job Details:
- Position: Medical Social Worker – Social Worker – Beverly & Addison Gilbert Hospital
- Certifications, Education, & Experience: A Masters degree in Social Work (MSW); current Massachusetts Social Work licensure (LCSW/LICSW); a minimum of one year of related experience in a hospital environment or with transferable skills applicable to a fast-paced, high-urgency setting.
- Schedule: 8-hour shift, Days, with weekends, 40 hours per week.
- Responsibilities: Perform psychosocial assessments, provide supportive counseling, and coordinate discharge planning and services. Collaborate with R N case management, clinical teams, and external agencies to ensure comprehensive patient care.
- Additional Information: Medical Social Worker must have experience with crisis intervention, protective services, child welfare, long-term care, hospice care, substance use disorders, and psychiatric illnesses is a plus. Flu vaccine required during onboarding. A hospital-based background and ability to triage crises are essential. Cannot have worked perm/travel/PRN for any Beth Israel facility within the past 6 months.
