Medical Surgical Cardiac Floor Care Coordinator (MS Cardiac) — Boston, Massachusetts
The organization is seeking an RN Care Coordinator to support patients on a structural heart unit with a strong focus on care coordination and triage. The role is primarily administrative compared to a typical RN position, including coordination of care, referred follow-ups, and administrative support while supporting the unit’s patients. Candidates are required to bring their license on the first day.

Job Details

  • Schedule: 10-hour days; 40 hours per week
  • Required Qualifications: Basic Life Support (BLS) (AHA); Epic experience; triage experience; 2+ years in care coordination (2 years in coordinator role required); 2 years of care coordination experience (2+ years required as stated); Level 1 trauma experience; Excel experience; strong communication skills
  • Preferred Qualifications: 4 years in a care coordinator role; strong administrative coordination/computer skills consistent with coordination responsibilities; structural heart and cardiac experience (structural heart and cardiac experience not required but preferred); 8-hour or 12-hour shifts as needed by the candidate (8–6 highly preferred)
  • Responsibilities: Coordinate care for patients on the unit; perform triage; complete administrative duties associated with care coordination, including referred and follow-up activities; support care coordination workflows while using Epic systems; provide coordination and follow-through for patient needs
  • Additional Information: BILH coversheet required at time of submission. No nights, holidays, or weekends. Orientation is weekly on Mondays. No local travelers accepted (cannot reside within 50 miles of the facility). All time-off requests must be presented at time of submission. Travelers who have worked for the organization as permanent staff or travel/per diem within the last 6 months are not accepted; candidates must be separated from the facility for 6 months to be considered.
