Job Summary:
The Roanoke, Virginia-based hospice care provider is seeking an RN Care Manager to join their team. This position involves managing patient care, providing symptom management, crisis intervention, and family education in primarily patient residences. Candidates are required to bring their license on the first day and must demonstrate compassion, excellent communication skills, and the ability to work collaboratively with an interdisciplinary team.
Job Details:
- Location: Roanoke, Virginia
- Schedule: Days, 8-hour shifts, minimum of 40 hours per week
- Required Qualifications: Virginia RN license; 1+ year experience in hospice care; valid driver’s license; good driving record and reliable transportation

- Float: Float rotation may be required.
- Additional Information: Travel to long-term care facilities, hospitals, and area agencies and businesses is necessary within the entire hospice service area. Overnight travel outside the service area may be required for continuing education. The employee is at direct risk for exposure to infections and blood borne pathogens.
