Job Summary: Home Healthcare Registered Nurse (HH), Morristown, New Jersey — The organization is seeking a Registered Nurse to administer skilled nursing care to home health and/or hospice patients of all ages. This role includes assessing patients in stable and emergent situations, coordinating care with the interdisciplinary team, and developing and evaluating a patient-centered Plan of Care (POC). Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days, 8a - 4:30p (Lunches deducted); 7.5 hours per shift; minimum 37.5 hours per week
- Required Qualifications: Current state license as a Registered Nurse; proof of current cardiopulmonary resuscitation (CPR) certification; certification in hospice and palliative nursing (CHPN) (preferred); Homecare Homebase software experience (required); OASIS experience (required); Education: graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (required); Bachelor of Science in Nursing (preferred); Minimum experience: 1–2 years as a Registered Nurse in a clinical care setting (required) and 2+ years home health experience (required); Knowledge and skills including venipuncture and intravenous (IV) care; proficiency with medication administration; and ability to assess and respond to patient and family needs in varied settings
- Preferred Qualifications: Bachelor of Science in Nursing; hospice and palliative nursing (CHPN) certification; additional hospice/home health experience beyond required experience, as applicable
- Responsibilities:
- Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs using clinical documentation, clinical record, case conference, team reports, and evaluations
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care
- Gather and incorporate physical, psychological, social, spiritual, and cultural factors that influence patient and family health status into the POC
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are accurately documented and updated as changes occur
- Provide care using infection control measures that protect staff and patients
- Educate patients and/or their families on care, diet, medications, home safety, and self-care practices using teaching methods appropriate to age, condition, culture, and learning readiness
- Supervise and provide clinical direction to the health care team to ensure quality and continuity of service
- Assess and reassess pain, use appropriate pain management techniques, and educate the patient and family regarding pain management
- Arrange referrals to appropriate agencies when needs exist
- Monitor assigned cases to ensure compliance with third-party payer requirements
- Provide on-call responsibilities and on-call services as assigned
- Communicate effectively as a liaison among patients/families, the interdisciplinary team, administration, physicians, and other health care professionals
- Attend interdisciplinary team meetings and other meetings as necessary and appropriate
- Perform other related duties as assigned
