Job Summary
Registered Nurse, Case Management, Emeryville and San Francisco, California. This position supports complex case management across the healthcare system and in the community through primarily virtual services. Responsibilities include discharge planning, care coordination, utilization review, and collaboration with interdisciplinary support services.
Job Details
- Schedule: Five 8-hour shifts per week, 8:00 AM–4:30 PM, for a minimum of 40 hours per week; two days onsite and three days remote.
- Required Qualifications: California-licensed Registered Nurse; Basic Life Support (BLS) certification; minimum of 2 years of case management experience; inpatient case management and discharge planning experience; community hospital experience; travel experience; electronic charting experience; experience supporting adult and geriatric patients.
- Preferred Qualifications: Previous charge experience; Level I or Level II trauma experience.
- Additional Information:
- Supports complex cases across the healthcare system and patients in the community.
- Primarily provides services virtually, with onsite work as needed.
- Performs admission criteria review, concurrent review, continued stay review, retrospective review, medical necessity review, prior authorization, care coordination, disease management, needs assessment, and plan-of-care development.
- Supports discharge planning, benefits eligibility review, utilization of InterQual criteria, and coordination of durable medical equipment.
- Collaborates with interpretation services and social services.
- Applies Health Insurance Portability and Accountability Act (HIPAA) guidelines and relevant regulatory and quality standards.
