Job Details:
- Location: Boston, Massachusetts
- Schedule: Day shift, Monday through Friday, 10 hours per shift, minimum 40 hours per week
- Required Qualifications: At least 2 years of case management experience, including 6 months of discharge planning, in an acute care hospital setting; candidates should have experience in large organizations or teaching hospitals with 500 beds or more; license or certification in case management must be current.
- Preferred Qualifications: Experience with pediatric and obstetric case management is appreciated but not required.
- Float: Float rotation may be required.
- Additional Information: Candidates must have relevant experience in acute care settings, ideally within large or teaching hospitals, demonstrating proficiency in discharge planning and care coordination.
