Outpatient Physical Therapist (PT OP) — Located in the healthcare organization’s outpatient setting, this position involves evaluating patients, developing, implementing, and modifying individualized treatment plans to optimize functional recovery and promote effective return to community activities. Job Details
- Position Description: Evaluates assigned patients and creates tailored treatment plans aligned with patient needs, goals, and physician directives. Responsible for the delegation and supervision of therapy services for assigned patients. Ensures the design of functional programs to support optimal patient outcomes. Serves as an educator for patients, families, students, and colleagues. Performs duties related to patient care, therapy service development, and program design. Demonstrates alignment with the organization’s Mission Statement and Core Values, maintaining professional standards, confidentiality, and compliance with organizational Code of Conduct.
- Certifications, Education, and Experience: Degree in Physical Therapy from an accredited institution; CPR certification; Active and applicable state licensure; 1+ year of outpatient clinical experience; No minimum years of experience required.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Additional Responsibilities: Completes comprehensive patient evaluations, develops and adjusts treatment plans using clinical reasoning, and ensures medical record documentation is clear, concise, and compliant with regulations. Participates in student education, maintains licensure and professional development, and contributes to performance improvement initiatives. Follows state practice acts and regulatory standards, and maintains ethical conduct.
