Job Details
- Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; active applicable license for the state where work will be performed.
- Responsible for performing comprehensive patient evaluations and developing individualized treatment plans aimed at progressing patients toward discharge within an established length of stay.
- Completes medical record documentation accurately, including goal setting, functional assessments, letters of medical necessity, and charge entry, in compliance with regulatory guidelines and organizational policies.
- Provides supervision and mentorship to students, fostering a positive learning environment.
- Actively pursues ongoing professional development through continuing education and engagement with professional organizations.
- Ensures all care is delivered under signed, valid physician orders and within insurance authorization parameters.
- Adheres to the organization’s performance improvement initiatives, contributing feedback to maximize quality outcomes.
- Demonstrates adherence to all safety policies, including safe patient handling and infectious disease protocols.
- Maintains professional licensure in good standing and completes all state-mandated continuing education requirements.
- Demonstrates knowledge of the organization’s performance improvement methodology and applies it to daily practice.
