Job Summary:
Outpatient Physical Therapist (PT OP) position available in Mesa, Arizona. Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Degree in Physical Therapy from an Accredited School, 0+ Years of Outpatient Clinical Experience, CPR Certification, active applicable license for Arizona, and successful completion of an assignment with the agency or organization (highly desired)
- Additional Information: Responsibilities include completing comprehensive patient evaluations, creating and modifying treatment plans, documenting medical records accurately and promptly, and participating in student programs and professional development activities. The role requires compliance with State Practice Act and other regulatory standards, ethical practice, and active involvement in organizational performance improvement initiatives.
