Job Details
- Position Description: Evaluates assigned patients and creates individualized treatment plans focused on patient needs and goals. Manages therapy services for assigned patients and ensures programs promote functional recovery. Serves as an educator for patients, families, and coworkers.
- Certifications and Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; outpatient clinical experience of six months to one year; Active state licensure in good standing; Experience with outpatient therapy.
- Schedule: Day shift, 8-hour shifts, minimum of 40 hours per week.
- Responsibilities: Conduct comprehensive patient evaluations, develop treatment goals, document medical records accurately and timely, and ensure care is provided in accordance with signed physician orders and insurance approvals.
- Additional Information: Participates in student programs, maintains professional licensure, pursues ongoing professional development, and complies with regulatory standards. Demonstrates strong clinical reasoning, adherence to safety and organizational policies, and engagement in performance improvement initiatives.
- Float: Float rotation may be required.
