Job Summary:
Physical Therapist Outpatient (PT OP) in a day shift setting. This position involves evaluating patients, developing and implementing treatment plans, and ensuring functional recovery to support patients' return to community activities. The role encompasses patient education, documentation, and adherence to clinical standards within a healthcare organization committed to safety and quality care.
Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: Degree in Physical Therapy from an accredited school, 1+ year of outpatient clinical experience, CPR certification, Ohio licensure, and a minimum of 1 year of experience in a similar setting.
- Preferred Qualifications: None specified.
- Float: Float rotation may be required.
- Additional Information: Completes comprehensive patient evaluations and develops goal-oriented treatment plans focusing on patient discharge within an established length of stay. Maintains accurate, timely medical record documentation, including goal writing, functional assessments, letters of medical necessity, and charge entry in accordance with organizational policies and regulatory guidelines. Participates in student teaching and ongoing professional development, adhering to State Practice Act and other regulatory standards. Demonstrates commitment to performance improvement and safety initiatives, including proper patient handling, infectious disease control, and hazard awareness.
