Job Summary
A Physical Therapist specializing in Home Health services is sought for a position in a healthcare organization providing home-based therapy. The role involves managing approximately six patient visits per day for adult patients with chronic diseases, wound care, IV therapy, pleurx drainage, and wound vac care. The position begins on 08/24/2026 and provides a daytime shift with a minimum of 40 hours per week.
Job Details
- Schedule: Day shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: Bachelor's Degree in relevant field; State Licensure as a Physical Therapist; BLS certification; minimum of 1 year of experience in home health therapy; valid driver’s license with proof of insurance; charting experience with Homecare Homebase (no exceptions); assessment experience with OASIS-E; experience with IV, pleurx, wound vac, wound care, disease teaching; proficiency in geriatric/Adult and chronic disease management.

- Additional Information: The assignment involves work within York County and possibly neighboring counties such as Lancaster, Union, or Mecklenburg. Travel may involve driving up to 100 miles depending on the caseload and patient location. Orientation will be conducted over 5 days. Scrub color required: Grey. The position is focused solely on home health care, requiring the use of Samsung Note 9 phones and the Homecare Homebase electronic medical record system.
