Job Details
- Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Massachusetts Physical Therapist Professional license, 1 year of Home Care clinical experience, 1 year of Case Management experience.
- Additional Information: Candidates must have a minimum of 1-year Certified Home Care experience; hospice, private care, or standalone Case Management roles do not meet the requirement. The start date is contingent on credentialing clearance; if not cleared by the scheduled start date, a reschedule will be necessary. Geographic flexibility includes coverage in multiple locations throughout the specified region.
