Job Summary:
The organization is seeking a Physical Therapist specializing in Home Health in the specified region. Job Details:
- Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Valid state license as a Physical Therapist, minimum of 1 year of experience in home health, and 1 year of case management experience. Experience in hospice, private care, or solely as a case manager without home health agency experience does not meet the qualifications. Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks. No experience minimum is listed apart from the specified years.

The start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled start will be necessary.
