Valid Physical Therapist (PT) license in Massachusetts. At least 1 year of Certified Home Care experience and 1 year of Case Management experience. - Schedule:
Shift Time: Days. Shift Hours: 8.00 hours. Minimum Hours Per Week: 40.00 hours. - Additional Information:
Candidates should have experience specifically within a certified home health agency, excluding hospice, private care, or independent Case Manager roles. Weekend rotations are scheduled once every 6 weeks. The position involves traveling to patient cases as assigned. Certification in electronic documentation, collaboration with healthcare teams, and compliance with agency standards are essential. - Float:
