Job Summary: Physical Therapist (Outpatient) — Newton, Massachusetts
The organization is seeking a Physical Therapist to provide physical therapy services to patients in their place of residence. Patients may include individuals from birth to over 100 years old. The role involves collaborating with patients, families, physicians, and allied health professionals, and completing thorough and timely electronic documentation and interdisciplinary communications in accordance with applicable policies and standards of practice. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days shift (8 hours) with a minimum of 40 hours per week.
- Start Date: 07/19/2026 (subject to credentialing clearance).
- Required Qualifications:
- Physical Therapist Professional license in Massachusetts
- 1 year of home care experience (certified home care experience specifically required)
- 1 year of case management experience
- Preferred Qualifications: None listed.
- Additional Information:
- Weekend rotations are once every 6 weeks.
- Ability to travel to cases as assigned is required.
- Experience in hospice, private care, or working as a case manager on its own does not meet the qualifications for certified home health care management experience; only roles specifically within a certified home health agency are considered relevant for the 1-year requirement.
