Physical Therapist (PT HH) — Newton, Massachusetts
The Physical Therapist (Home Health) will provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. The role involves collaborating with patients, families, physicians, and allied health professionals to identify needs and coordinate services, while maintaining thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies and standards of practice. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Days shift; 8 hours per shift; minimum 40 hours per week.
- Start Date: 08/09/2026 (start date is based on credentialing clearance; if not cleared by the posted start date, a rescheduled start date will be necessary).
- Required Qualifications: Physical Therapist Professional license in Massachusetts; 1 year home care experience; 1 year certified home care experience (experience in hospice, private care, or working as a case manager on its own does not meet the qualifications); 1 year case management experience; ability to travel to cases as assigned; weekend rotations are once every 6 weeks.
