Job Details

- Required Qualifications: Valid Physical Therapist Professional license in Massachusetts; 1 year of experience in a home care setting; 1 year of Certified Home Care experience; 1 year of Case Management experience.
- Preferred Qualifications: Experience in hospice, private care, or working as a Case Manager outside of a certified home health agency does not meet the required qualifications.
- Additional Information: The role requires travel to cases as assigned and weekend rotations approximately once every 6 weeks. To be considered, candidates must demonstrate the ability to collaborate with a multidisciplinary team and maintain thorough, timely electronic documentation in accordance with agency policies and standards of practice.
