A licensed Physical Therapist (PT) specializing in Home Health is needed in Medford, Oregon. This position involves delivering skilled physical therapy services to patients within a home health setting. Candidates are required to bring their license on the first day. The role begins as soon as possible and requires availability for 8-hour daytime shifts.

- Preferred Qualifications: Home Health experience preferred.
- Additional Information: Candidate must have reliable transportation. Experience with Electronic Patient Care Records (EPIC), Oasis, and meeting patient care point expectations is preferred. The position involves working with adult patients across various diagnoses, including post-operative conditions, neurological and spinal injuries, and general mobility rehabilitation. This role does not include on-call requirements or weekend work.

- Vaccinations: COVID vaccination booster is required.
- Experience: 1 year in Skilled Home Health setting desired.
- Travel and Flexibility: All travelers are expected to float between all facilities within the state; refusal to float may result in termination or forfeiture of guaranteed hours.
