Job Details
- Certifications, Education, and Experience: BLS certification required; active license in hand; minimum of 1 year of relevant experience including OASIS experience for staff completing Start of Care (SOC) in home health; proficiency in Homecare Homebase (HCHB) required.
- Schedule: 8-hour shifts, 40 hours per week, days shift, starting 02/01/2027 with an end date of 05/03/2027.
- Responsibilities: Providing pediatric and adult home health therapy focused on geriatric patients, performing assessments, documentation, and patient education.
- Additional Requirements: Must have active driver’s license and personal transportation; on-call rotation may be required; timely documentation and communication essential; Monday orientation required; RTO (Requested Time Off) must be submitted at application; county-specific territory flexibility required.
