Job Details
- Specialty: Physical Therapist Home Health (PT HH)
- Schedule: Monday through Friday, 8-hour shifts, with a minimum of 40 hours per week
- Experience Requirements: Minimum of 1 year of relevant experience, including OASIS experience for staff completing Start of Care for Home Health, proficiency in Homecare Homebase (HCHB), active driver's license, and personal transportation required.
- Certifications: BLS certification required
- Additional Requirements: License in hand, active driver's license, timely communication and documentation, and submission of Requested Time Off at application.
- Responsibilities include delivering therapy services to geriatric patients, completing modules during orientation, and participating in on-call rotations (discussed during interview).
- The role involves traveling within up to a 60-mile radius from the hub/office, with travel pairs accepted but each traveler must have their own transportation.
