Job Summary:
The healthcare provider is seeking a licensed Physical Therapist specializing in Home Health services to serve in the Hyannis, Massachusetts area. Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Bachelor's degree in Physical Therapy from an accredited Physical Therapy Program; licensed to practice in the Commonwealth of Massachusetts; current Basic Life Support (BLS) certification; ability to read, write, and communicate effectively in English
- Preferred Qualifications: Documentation of Specialist Certification; experience in home health clinical settings; ability to serve as a clinical resource and provide supervision or training as needed
- Additional Information: The role includes evaluating patients, setting measurable treatment goals for functional outcomes, administering diagnostic muscle, nerve, joint, and functional ability tests, and carrying out appropriate treatment modalities. The therapist will also create documentation reports, communicate effectively with the healthcare team, educate patients and families, and contribute to departmental quality improvement initiatives.
