Job Details
- Position: Physical Therapist (PT) Home Health
- Location: Seaside, OR and surrounding areas
- Shift: Days, 8-hour shifts, minimum 40 hours per week
- Schedule: 8 hours shifts during daytime hours, with a tentative start date of 11/16/2026 and end date of 02/15/2027
- Required Qualifications: BCLS certification, active driver’s license, personal transportation, minimum of 1 year of relevant experience in home health, proficiency in Homecare Homebase (HCHB), OASIS experience REQUIRED for staff completing Start of Care, and adherence to facility vaccination requirements (COVID-19 primary series and booster)
- Additional Information: Timely communication and documentation are mandatory; RTO requests must be submitted at application; on-call rotation may be required; travel involves occasional maximum radius of approximately 60 miles per day; familiarity with electronic charting via Home Care Homebase and Rover app is essential
- Submittal Requirements: Active license in-hand, submission on Monday orientation day, and compliance with all travel and licensing regulations
- Preferences: Home health experience, OASIS competency, and ability to work independently in patient’s homes
