Job Details:
- Location: Clear Lake, California
- Schedule: Day Shift, 8 hours per day, Monday through Friday, no weekends, minimum of 40 hours per week
- Required Qualifications: Doctor of Physical Therapy (DPT) degree obtained on or after January 2016, or foreign equivalent; valid Physical Therapy licensure in all applicable states of practice; current AHA-BLS or AHA-CPR certification; valid Driver’s License and being at least 21 years of age or older; at least 1 year of home health experience; proficiency with HCHB and OASIS electronic medical record systems
- Additional Information: Must have knowledge of HCHB and OASIS systems; vaccinations required per organization policy and annually thereafter; E-Verify participation applies
