Pharmacist Inpatient (PHARM IP) — Located at the healthcare facility, this position involves providing safe, effective, and cost-effective pharmacotherapy to inpatients. Pharmacists support medication-use systems, participate in medication management, and serve as key resources within the healthcare team to improve medication safety and patient outcomes.
Job Details
- Position Description: Responsible for interpreting and evaluating physician orders and prescriptions, preparing, calculating, and dispensing appropriate medication dosages. Collaborates with healthcare professionals to ensure optimal medication therapy and participates in medication safety initiatives. Supports pharmacy operations, including supervising pharmacy technicians, interns, and ancillary personnel.
- Required Qualifications: Graduate of an ACPE-accredited College of Pharmacy; B.S. in Pharmacy or Doctor of Pharmacy (PharmD) degree; Current licensure as a Pharmacist in practicing state; 2 years of experience; Licensure, Certifications, and Clearances: Pharmacist License PA, Act 31, PA Act 33, PA Act 34, PA Act 73.
- Schedule: Rotating shift schedule with 8-hour shifts, minimum 40 hours per week.
- Additional Information: Pharmacists will provide medication and practice-related education, serve as preceptors for pharmacy students and residents, and participate in quality improvement initiatives aimed at enhancing clinical pharmacy services.
