Inpatient Pharmacist - PHARM IP — Located in an acute care facility, this role involves providing comprehensive clinical pharmacy services within an inpatient setting. Job Details
- Position Description: Clinical Pharmacist — Full-time of 40 hours per week, reporting to a 5-day schedule with eight-hour shifts. The role includes working one to two weekends per month and up to two of the three winter holidays (Thanksgiving, Christmas, New Year's Day).
- Required Qualifications: Valid pharmacist license, experience with Meditech system, at least 2 years of inpatient pharmacy experience, proficiency in acute care pharmacy clinical services including vancomycin, warfarin, heparin therapy management, antimicrobial stewardship activities, and participation in interdisciplinary team rounding.
- Schedule: Days, 8 hours per shift, minimum 40 hours per week.
- Additional Information: The successful candidate will be responsible for medication therapy management and collaborating with interdisciplinary teams to support patient care initiatives.
