Job Details
- Schedule: Days, 8-hour shifts; minimum 40 hours per week.
- Required Qualifications: Current Oregon (OR) license and Washington (WA) license are required; Basic Life Support (BLS) certification; 1 year of skilled home health experience.
- Preferred Qualifications: Oasis documentation experience.
- Responsibilities: Provide occupational therapy care for patients including assessment, treatment planning, reassessments, and safety/injury prevention. Support functional activities and patient/family education, including discharge planning and community re-entry needs. Utilize remote home health workflow tools (including laptop-based and mobile applications) for documentation.
- Certifications: BLS.
