Job Summary:
Long Term Care Licensed Practical Nurse (LPN) position in Spring Grove, Minnesota. This role involves providing care in a 43-bed long-term care facility, with clinical responsibilities aligned with the facility’s mission. Candidates are required to bring their active Minnesota LPN license on the first day.
Job Details:
- Location: Spring Grove, MN
- Schedule: 8-hour/12-hour shifts (mostly 8 hours), variable PM/NOC shifts. Shift hours are rotating, with every other weekend and holiday required.
- Required Qualifications: Active Minnesota Licensed Practical Nurse (LPN) license, associate degree or Certificate, 2+ years of LPN experience, 1 year in long-term care, minimum 36 hours per week, minimum of 0 years of experience (additional preferred experience of 2+ years in LPN and 1 year in LTC).
- Preferred Qualifications: Experience with Matrix Electronic Medical Records system.
- Additional Information: COVID-19 vaccination is required; exemptions will be considered. The position entails floating to a nearby facility when needed. The clinical shift schedule involves at least one nurse and two certified nursing assistants (CNAs) during night shifts, with additional staff on days/evenings.
- Compliance: Candidates must initiate the medical portion of compliance within 48 hours of accepted offer.
