Long Term Care Licensed Practical Nurse (LPN) — The healthcare provider is seeking a dedicated Long Term Care LPN to join the team for a short-term assignment. The role involves assisting with patient assessments, administering medications, and providing person-centered care in a 120-bed facility, with a focus on interdisciplinary collaboration and maintaining clinical competence.
Job Details
- Position Description: The primary functions include assisting with assessment, planning, implementing, and evaluating patient care across all ages during the shift. The LPN collaborates with the care team, participates in interdisciplinary care, and advocates for patients, administering medications and treatments under RN supervision in accordance with facility policies.
- Required Qualifications: Valid LPN license, current health submission, and ability to work on the specified schedule.
- Schedule: Dayshift, 8-hour shifts, working 3 days per week from 3:00 PM to 11:30 PM. The assignment begins on 11/08/2026 and ends on 02/06/2027.
- Additional Information: Responsibilities include taking vital signs, observing and reporting patient condition changes, maintaining documentation in electronic medical records, and floating to other units as needed. Staffing ratios are 1 RN to 2 LPNs, with 4-5 CNAs per unit. Orientation consists of a 1-day on the assigned unit. Scrubs of any color may be worn. The role requires willingness to work every other weekend and holiday, with potential to serve as a CNA when staffing needs arise.
