Long Term Care (LTC) Licensed Practical Nurse (LPN) position in the area. This assignment involves providing dementia-capable care within a rehabilitation and nursing facility. Job Details
- Position Description: Tentative backfill for a 16-26 week assignment, with tiered rates based on direct line mileage. The candidate must have prior travel experience, EPIC Electronic Medical Record (EMR) experience from within the past 2 years, and a minimum of 1 year of related experience.
- Certifications and Education Requirements: Valid LPN license, dementia-capable care certification (preferred).
- Schedule: Days shift, 8-hour shifts, minimum of 40 hours per week.
- Required Qualifications: A minimum of 1 year of experience, prior travel experience, EPIC EMR experience within the past 2 years, and valid LPN licensure.
- Additional Information:
- IVS to offer.
- No block schedules, with bi-weekly orientations starting 10/12.
- Ability to work 2 out of 4 holidays.
- Ensure all employment gaps over 90 days are explained.
- Complete work history including bed count, EMR, and facility type is required for submission.
- Reliable transportation strongly recommended, with personal or rental vehicle equipped with all-season or snow tires during November through April.
- Float: Float rotation may be required.
