Job Summary:
Licensed Practical Nurse (LPN) for Long Term Care unit in Middlebury, Vermont. This position involves providing dementia-capable care, with a focus on patient-centered services, and requires previous experience in a similar setting. The role includes the opportunity for float coverage and requires working nights shifts.
Job Details:
- Schedule: Nights shift, 12 hours per shift, with a minimum of 36 hours per week.
- Required Qualifications: Valid LPN license, at least 1 year of experience, travel experience, EPIC electronic medical record system experience within the past 2 years, dementia capable care certification preferred.
- Preferred Qualifications: Prior travel experience, EPIC experience from within the past 2 years.
- Float: Float rotation may be required.
- Additional Information:
- The position requires a CNA must start within the next week, with a 7-year background check (nationwide, county court searches, FACIS 3, and sex offender registry) within 18 months of the start date.
- This role involves working Monday through Sunday with no block schedules and bi-weekly orientations on Mondays.
- All employment gaps over 90 days must be explained.
- Complete work history (bed count, EMR use, facility type, etc.) required for submission.
- Reliable transportation is strongly recommended as public transportation is not accessible in this area; personal or rental vehicle with all-season or snow tires during November to April is required.
