Long Term Care (LTC) Licensed Practical Nurse (LPN) – Evening Shift — This position offers a vital role within the healthcare organization providing patient care in a long-term care setting. The nurse will be responsible for admissions, discharges, transfers for acute care, and clerical duties, primarily working evening shifts with rotations on weekends and holidays.
Job Details
- Position Description: Evening, 8-hour shifts from 2:30 PM to 11:00 PM, with every other weekend rotation and some holidays. Responsibilities include admission, discharge, transfer to hospital for acute care, and clerical duties.
- Required Qualifications: WI Licensed Practical Nurse (LPN) or compact license, BLS certification from the American Heart Association, at least 1 year of experience as a Long Term Care Licensed Practical Nurse, and flu vaccination.
- Schedule: Evening shifts from 2:30 PM to 11:00 PM, minimum of 40 hours per week.
- Additional Information: The candidate should have experience with patient admission, ambulation, medication management including antianxiety, antidepressants, antihypertensives, and antipsychotics, as well as skills in behavior management, BiPAP/CPAP, blood glucose monitoring, and wound care. Knowledge of electronic charting systems and prior LTC experience is preferred. The role involves close patient interaction and adherence to safety protocols, including PPE and isolation precautions.
- Certifications: BLS from AHA, WI LPN or compact license.
- Submission Requirements: Candidates must meet minimum experience requirements and successfully complete pass/fail testing with a telemetry exam prior to start, demonstrating proficiency in identifying lethal rhythms and heart blocks.
