Job Summary

Job Details
- Required Qualifications: Active Oregon license; minimum of 1 year of experience in skilled home health; proficiency in Homecare Home Base (HCHB) electronic documentation system; at least 1 year of relevant work experience; BLS certification.
- Schedule: 8-hour day shifts, 40 hours per week.
- Additional Information: Clinicians must have reliable personal transportation, and the service area may extend up to approximately 60 miles or 60-90 minutes of total daily driving. Home Care Home Base (HCHB) experience is required. All travelers are required to have an active driver’s license and personal transportation. RTO (Requested Time Off) must be submitted at the time of application. On-call rotation may be required.

- Documentation and Technology: Use of Homecare Homebase (HCHB) system and Rover phone application.
- Special Requirements: Clinicians should have at least one year of skilled home health experience and be able to work weekend support if needed.
