A short-term acute care facility is seeking a Medical Social Worker for a 13-week assignment. This role involves providing behavioral health assessments and care management discharge planning across various units, including Emergency Departments and medical/surgical floors, with the ability to float between multiple facilities as needed.

- Shift: Rotating, 10 hours per shift, with a minimum of 40 hours per week

- Required Qualifications: Bachelor’s Degree in Social Work, current BLS certification, valid state license (must be a compact license in hand at submission), at least 1 year of experience in behavioral health assessments and care management, ability to float between units including Emergency Department, medical/surgical floors, and community health settings.
- Preferred Qualifications: Experience in acute behavioral health assessments in emergency settings and care management discharge planning.
- Additional Information: The candidate must have the ability to perform behavioral health evaluations on all ages, participate in multidisciplinary care conferences, and coordinate services for patient discharge and community resources. Candidates should be flexible to rotate shifts and work an open schedule, including some holidays.
- Submission Requirements: Candidates need to respond to whether they have worked at a healthcare facility as a permanent employee, traveler, or per-diem, including details. Two references covering one year within the last three years are required, and references should include Charge RNs or Lead Travelers.
