Job Summary:
A healthcare organization is seeking a Social Worker for their hospice team in a high-quality care setting. This position involves providing biopsychosocial assessments and support for patients of all ages, with the majority being over 65 years old. The role is focused on delivering compassionate social work services within a 8:00 AM to 4:30 PM schedule.
Job Details:
- Position: Social Worker (MSW or working towards Licensed Clinical Social Worker)
- Location: In a hospice setting, serving a diverse patient population.
- Required Qualifications: Masters in Social Work (MSW), active driver’s license, active Basic Life Support (BLS) certification, minimum of 2 years of clinical, emergency department, or medical social work experience. EMR system is Epic.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week, starting on 11/15/2026 and ending on 02/14/2027.
- Responsibilities include completing biopsychosocial assessments, supporting patients and families, and lifting 50 pounds intermittently.
- Additional Information: All patients are cared for in a hospice environment; no on-call responsibilities. Must have a vehicle capable of driving in various road conditions, including gravel, snow, rain, and ice. The department has a lead social worker. The position may involve the use of any color scrubs. The applicant must be prepared for a prescreen interview, with final interview and offer conducted by the manager.
- Float: Float rotation may be required.
