Job Summary
Physical Therapist Outpatient (Specialty: PT OP) – NovaCare Rothman-Chalfont. This outpatient position involves evaluating patients, developing and implementing treatment plans, and ensuring safe, effective therapy services to promote patient recovery and functional return to community activities. The role requires collaboration with patients, families, and healthcare team members to deliver high-quality care.
Job Details
• Certifications, Education, and Experience Requirements: Degree in Physical Therapy from an accredited school; 2 years of outpatient clinical experience; CPR certification; active license for the state where work will be performed.
• Schedule: Dayshift, 8-hour shifts, with a minimum of 40 hours per week.
• Responsibilities: Conduct comprehensive patient evaluations, develop individualized treatment plans, document medical records accurately and timely, ensure clinical care aligns with physician orders and insurance approvals, and participate in student training programs.
• Additional duties: Maintain professional licensure in good standing, engage in continuous professional development, adhere to state practice acts and organizational standards, and participate in performance improvement initiatives.
• Preferred Qualifications: Experience with clinical program development, familiarity with organizational performance improvement methodologies, prior successful assignments with the agency or Select Medical.
• Float: Float rotation may be required.
