- Experience Required: 3 years of experience in social work, particularly with hospice and home health patients
- Schedule: Days shift, Monday through Friday, 8-hour shifts, 40 hours per week, with a weekly interdisciplinary team meeting on Thursday mornings
- Additional Information:
- Must have Clinical Social Worker (LCSW) license and BLS certification
- Experience covering hospice services and home health
- EMR system used is Homecare Homebase
- Must be able to assist with Medicaid applications and nursing home placements as needed
- No requirement for BLS beyond in-house training
- Dress code is business casual
- Holiday hours may vary, with closures on major holidays
- Flexibility to work weekends if needed and available
- The role includes clinical oversight and guidance from a full-time MSW colleague
