Job Summary
Home Health Physical Therapist (PT HH) – Mt. Angel, OR and surrounding areas. This position requires a licensed Physical Therapist with a minimum of one year of experience in skilled home health settings. The role involves providing therapy services to patients in their homes, ensuring quality care in line with clinical and operational guidelines, and working within the community to support patient recovery and independence.
Job Details
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: Valid license in hand, minimum of one year of skilled home health experience, proficiency in Homecare Homebase (HCHB), and OASIS experience required for staff completing Start of Care (SOC). Must have an active driver’s license and own transportation.
- Preferred Qualifications: Experience with community-based therapy and strong documentation skills.
- Clinical Responsibilities: Provide skilled physical therapy services in patients' homes, perform patient assessments, develop and implement treatment plans, and document care using remote charting systems.
- Additional Information: Occasional maximum radius of roughly 60 miles per day; travel pairs accepted with individual territories; modules are billable and completed during orientation. RTO requests must be submitted at the time of application; on-call rotation may be required. Former employees must be separated for at least six months prior to reapplication and need HR approval.
