Job Summary
Physical Therapist Home Health (PT HH) — Located in Morris County, this position involves delivering exceptional home health care within a large acute care setting. The role requires a team-oriented professional capable of evaluating patients, establishing care plans, providing interventions, and educating patients and their families, all while maintaining accurate documentation and billing.
Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: New Jersey License to practice Physical Therapy; Graduate of an accredited Physical Therapy academic program; 0-4 years of experience as a Physical Therapist is strongly preferred.
- Schedule: 8 am to 4 pm (Monday-Friday), with a minimum of 40 hours per week.
- Preferred Qualifications:
- Experience in Acute Care within a large academic medical setting.
- Successful clinical affiliation with Atlantic Health.
- Board Certification as Specialist in an area suitable for care in an acute care setting.
- Additional Information:
- Home health experience required.
- Valid Physical Therapist license required.
- Epic experience preferred.
- Support our Morris County region.
- Participate in orientation of new staff and serve as a clinical instructor for students.
- Exercise independent judgment in moderately complex cases with minimal supervision.
