- Preferred Schedule: Schedule would be Monday-Thursday 7:45-3:15 and Friday 7:30-12:30 Dates needed are 7/15-7/24 Please ONLY submit candidates available all dates 7/15-24!
- Worksite Setting: Outpatient. Patient load is 10-15 average daily M-F
- Scope of Work: Provider will need to complete the weekly treatment visit as well as oversee the treatments.
- Licenses, Certifications, Requirements: Active LA License, BC, ACLS, BLS, Fellowship: Preferred
