- Work Dates Needed: ASAP - 3 months
- Preferred Schedule: Monday through Friday 8a-5p | 1 in 4 call rotation which is all phone based
- Worksite Setting: PACE outpatient center based with some in-home visits
- Scope of Work: Diagnosis and treat nursing home eligible senior population who desires to remain living independently, function as part of the interdisciplinary team to care plan for agreed upon participant goal outcomes | 5 to 10 patients a day
- Licenses, Certifications, Requirements: Michigan licensure, ACLS/BLS, geriatric population experience required, PACE experience required
- EMR: CollabriOS PACE Care Online (PCO)
- Rates: $90/hour
