Position Overview:
- Facilitate referrals and provide consistent follow-up to ensure families access necessary support.
- Assist families in navigating applications for medical assistance and help parents understand their child’s specific developmental needs.
- Align efforts between external clinics, private therapists, and school staff to ensure a cohesive approach to student well-being.
- Partner with multidisciplinary teams to develop and implement integrated support plans for students.
Qualifications:
- Master’s degree in Social Work (MSW) or Social Welfare
- Valid Pennsylvania Social Work License and Department of Education Certification
- Prior experience in counseling, family engagement, and accessing mental health services, with a strong familiarity with special education frameworks
